Field Name Description

Injury Types list

The type(s) of injuries sustained by the subject. For example, Gunshot Wound.

Condition list

The current condition of the subject. For example, Impaired / Alcohol

Medical Treatment list

The treatment performed or required for the arrestee. For example, Transported or Treated at Scene

Hospital list

The hospital to which the arrestee was transported.

Transported By list

The method of transportation used when transferring the arrestee to a hospital, if applicable. For example, Fire / EMS or Air

Extent of Injury list

The severity of the injury. For example, Mild or Fatal

Degree of Sexual Assault list

The degree of sexual assault committed or experienced by the subject. 

Number of Previous Complaints text box

The number of complaints of domestic violence previously made about the subject.

Federal Agencies Involved check box

Select this check box if any federal agencies are involved in the case.

Examples of federal agencies that may be involved in a case include the Federal Bureau of Investigation (FBI), Drug Enforcement Administration (DEA), Bureau of Alcohol, Tobacco, and Firearms (ATF), and the Center for Disease Control and Prevention (CDC).
